What is Hospice?

Hospice care is a coordinated program of palliative and compassionate care provided by a team of skilled professionals to meet the medical, psychosocial, spiritual needs of patients and families who are facing terminal / chronic illnesses for which they elect comfort care rather than aggressive treatment. Hospice care is palliative (comfort / symptom oriented) rather than curative.

The goal of Hospice is to empower the individual by providing knowledge and information to facilitate difficult decision-making. Our clinical staff consists of experts in pain control, and other symptom management. They are commited to educating patients, caregivers, and family members

Where Does hospice Care Take Place?

Hospice Care is primarily based in the patient's home or home-like setting such as a nursing home or board and care. Hospice enables families to remain together in comfort and dignity. If the patient needs to be hospitalized for control of acute systems in a nursing home, Hospice can continue to provide palliative care in those facilities.

What Are the Advantages of Hospice Care?

Hospice treats the person, not the disease. Care is family-focused and emphasizes promoting quality and dignity in the late stages of life.

Hospice uses a team approach by utilizing the individual expertise of the members of the interdisciplinary team to address the unique needs of the patient and their families.

Hospice Staff are 24 hours a day, 7 days a week

Hospice Provides Durable Medical Equipment; Hospital bed, bath chair, oxygen.

Hospice also provides medication related to disease and formulary medications. Hospice also provides soft goods as needed. Such as Diapers, bed chux, gloves.

Who is Eligible for Hospice Care?

Have a life-threatening illness and seek medical, psychological, spiritual, and personal support rather than aggressive treatment.

Have a Primary Care Physician.

Live within our geographic boundaries.

Who Pays for Hospice Care?

Medi-Cal, Medicare, health Insurance, HMO's, and Private Pay.
